How to Use:
- Print directly and write with pen/pencil, or
- Open PDF file in Adobe Reader (free software), fill in the blue boxes, then print
Important: Blue boxes will not appear on the printed version.
Editing Instructions:
(For computer use only – not compatible with phone/iPad)
- Open the PDF file in Adobe Acrobat Reader (free software)
- Type in the blue boxes
- To edit text:
- Highlight the desired text
- Press “Ctrl+E” (Windows) or “Command+E” (Mac)
- Use the toolbar to modify font, size, and color
Important Notes:
- Editable areas are limited to blue boxes only
- Other text and graphics cannot be altered
